Registering as a new patient
A map of the area our practice covers is shown below. When registering
we will ask you to complete a brief questionnaire for each member of
the family. It is very important that you complete this as it may take
some months for your medical records to reach us.
We would also encourage you to have a new patient medical with one
of our nurses so that we are aware of your current health status.
We operate a personal list system, encouraging you to see your own
doctor when possible. If your own doctor is unavailable you will be
offered an appointment with another doctor or clinical professional.

We would be most grateful if this form can be completed and either brought in,
sent to the Surgery, or emailed to us. Please bring the form with you when
attending your New Patient Medical.
