Diabetes
It is important that you are seen at least once a year if you have a diagnosis of pre-diabetes (non-diabetic hyperglycaemia) or type one/two diabetes.
If you are pre-diabetic, we will call you annually for a blood test to ensure your hba1c levels remain at an appropriate level. We can refer you onto other services if you would like additional support in weight management or healthy eating.
We run diabetic nurse led clinics where you will be invited in. This will involve a blood test and urine sample to send to the lab which will then be reviewed with you in this clinic. Please contact us 01822 612247 to book an appointment or visit our reception.
More information can be found here about the signs and symptoms of diabetes.

MENTAL HEALTH APPOINTMENTS
Mental Health appointments aged 18 and over
There are dedicated mental health appointments for our patients aged 18 and over. Our specially trained mental health nurses offer appointments throughout the week. They provide a single, longer appointment to explore your current circumstances, and consider the most appropriate support. They can introduce some self help strategies, services you can access independently, make onward referrals or liaise with GP regarding medication if this is needed. There may be occasions where it is felt a review would be beneficial- this will be directly booked or requested by the mental health nurses.
